I am having issues with a VLOOKUP formula in a benefits report where I am comparing data sets A and B using the identifier EE Number from Set A to pull in various data from Set B. Each employee is listed multiple times with different benefits and amounts. The formula is only providing the first two results and then duplicating the rest. How can I fix the formula to correctly pull different benefit codes for each employee?

My husband's account appears to have been hacked, and I can no longer log in. The email address and phone number linked to the account are ***@******.*** and (###) ###-####, respectively. When I try to reset the password, an email address that neither of us recognize appears. My husband passed away in 2016, and I had sole access to the account.
