He has been to the vet, where all lab tests were negative, bowel sounds were good, and he was given amoxicillin, an antiemetic, a probiotic, and fluids. However, he doesn't seem better and is now constipated. What could be wrong and what should I do?

One of my dog's toenails split all the way from the end up into the toe itself. It is not bleeding, but I can see the quick. The end closest to the toe is bright pink and healthy looking, but the end is grey. The dog does not seem to be in pain, but she does lick the affected toe and the one next to it. The dog's name is Sadie, she is 11 years old, and she has hip dysplasia and struggles with problems in her rear end.
