My dog, Cici, a 9-year-old Yorkie Maltese mix, has been experiencing severe itching on her paws and sensitive skin for months. She has been licking and rubbing the affected areas, and now has a crusty round patch on her chest. Her nails also show some crusting where the pad meets the nail bed. Additionally, she has been scooting on the floor, her tail is red, and her vulva has been infected multiple times. The vet has already tried prednisone, apoquel, and simplicef, but nothing has worked. What could be causing these symptoms and what further steps should be taken?
