I am 44 years old and have had erectile dysfunction since my teenage years, characterized by quick loss of erections and dependence on manual or oral stimulation. I have been using Cialis daily for 6 months, which helps with morning erections but not consistently. I suspect a venous leak and have an appointment for a doppler ultrasound. Additionally, I experience soreness at the base of my penis, small bumps on the shaft, and periodic shrinkage in penis girth, especially after bowel movements, which affects my ability to achieve and maintain an erection. I have low testosterone levels and am starting testosterone pellet therapy. What could be causing these issues and how might they be related?
