I have a hiatal hernia of the esophageal sphincter that is currently causing severe swallowing difficulties and regurgitation. I have been unable to swallow or keep anything down, including water, for over 20 hours. I have no insurance and cannot afford to go to the doctor's office or the ER. Is there anything I can do for this on my own?

Last month, my TSH was 129, and after doubling my synthroid dosage from .075mcgrms to .150mcgrms, my TSH is now at 89.90. The doctor has now put me on .200mcgrms of synthroid. What side effects should I look for, and should I be worried if my TSH does not go down?
