I had sudden shortness of breath on Christmas that worsened and led me to the ER. I had normal EKG, chest X-ray, blood tests, and urine tests. I have chest pain, throat tightness, and shortness of breath that persists for a week. I'm 22 years old, male, 195 pounds, no medications, and no prior medical history. What could be causing this and what should I do?

I suddenly had severe rectal pain after straining during a bowel movement five days ago. The pain has been severe and constant since then, and my perineum is also very painful. There is no bleeding, and it almost feels like something is twisted or a muscle is pulled. The pain is not getting worse or better after five days. Would a hemorrhoid cause this much pain? There is no indication of external hemorrhoids and no bleeding. The pain is not a sharp knife-like pain that I have had before with hemorrhoids but more like a very painful pulled muscle in the rectum, and the perineum is very tender to the touch. I have been sitting on a heating pad which makes it feel better temporarily, but whenever I move the pain is very intense. I also came down with the flu 2 days ago and have had lots of diarrhea and just today the stools turned black. I have taken Pepto Bismal and not eaten anything but chocolate pudding. Before when the problem occurred I was constipated. This problem occurred during the straining episode. If it is an internal hemorrhoid, what do I do to treat that?

I'm experiencing severe ear pain that extends to my jaw when I open it, and my ear is swollen inside. I've been taking acetaminophen for three days without relief. Could this be an ear infection, and what should I do?

Do you think this is the flu: chills and sweats for over 1 week (started 1/5/09), extremely fatigued, no congestion or cold symptoms, no fever (97.5), had hiccups and vomited for the first 3 days mostly bile, was brown in color before seeing a MD (who prescribed Vicodin, Levaquin, Promethazine, Protonix, and liquid to soft diet). Has a history of esophageal hernia and recurrent kidney stones (takes Allopurinol). Now consistently sore in the diaphragm area, has a hypersensitive gag reflex, and a hoarse voice. Had small bowel movement for the first time today 1/12/09, causing a gag reflex from pushing. Last BM prior to this was 1/6/09. Has mostly been forcing down small amounts of yogurt, soup, cream of wheat, shakes. Also has a lot of burping. Finally took the Vicodin 2x's today and the diaphragm is starting to "feel better".
