When they say your period starts, is it 28 days from the last day of your last period or the first day of your last period?

I am a 41-year-old female who experienced vaginal bleeding during sex. I've had a hysterectomy approximately 10 years ago, but I still have my ovaries and cervix. I've also had two C-sections and a tubal ligation. My last PAP smear was two years ago, and it was usually normal, but I know I've had ovarian cysts. Is this bleeding an immediate threat, and would it happen during every intercourse?
