A doctor has deleted medical conditions and included inaccurate information in a patient's medical record, such as stating there are no mobility issues despite the patient being wheelchair-bound for 7 years. The patient advocate at the Military Treatment Facility (MTF) has refused to correct the record. What steps can be taken to have the record amended, and is it necessary to involve an outside independent body like Congress to address this issue?
