I am sending a Notice of Service of Discovery to the courts. How do I reference the attorney in the document? Let’s say in the document I am stating I sent the documents to you. Do I say Matthew L. Esq or do I say Matthew L Esq council for the defendant?

I went to a dentist for a smile makeover and paid $28,000, but I'm not happy with the results. They promised a Hollywood smile but delivered something different. When I expressed my dissatisfaction, they dismissed me without resolving the issue. I have pictures and plan to post reviews online. I want my money back to redo it elsewhere. What can I do?
