I had a legal name change about 10 years ago. I want to remove the 'new' middle name portion only and restore my birth middle name portion. The courthouse clerk said I would have to file a whole new name change form again, including fingerprints, as amendments are only for mistakes on the original name change form. The Birth records division said only an amendment would be necessary. How do I do this? What words do I say?
