I filed a blanket recusal form against the trial judge, which was denied. I then filed a formal recusal, which was also denied, and the judge provided reasons for her decision. The judge allowed defendants to file pleadings without serving me and permitted them to pay fees after their answer was removed for non-payment. The judge also denied my requests to recall a defendant for impeachment, to play a bodycam video, to enforce subpoenas, and to grant continuances. Additionally, the judge denied my request for a court reporter, which impacts my ability to appeal. What are the implications of these rulings, and can I still claim judicial bias on appeal?

My question is: Who should be the defendant in this lawsuit? The insurance company, the policyholder, or both?
