I had a visit to The Little Clinic in December for an abbreviated physical for cataract surgery and was told they do not file claims with insurance companies, even though they are in-network. I had to pay upfront and filed the claim myself, but the CPT codes used by The Little Clinic do not match the insurance company's requirements. I was told by law they need to file the claim but they refuse. The insurance company told me I am not responsible to pay for that service and I called to get reimbursement, but they refused. Where do I stand and can you help me?

I don't know what type of case I have or which type of lawyer I need. My ex took my car title and added his name to it without my consent. The DMV in Nevada allowed this and now he refuses to remove his name. I have the bill of sale and a witness who sold the car to me. The DMV supervisor destroyed the original documents and microfiched them. They told me I need a judgment to get my title back. What steps should I take and what type of lawyer do I need?
