I had an affair with a married man for 2 1/2 years in TN. He is a trusted public figure and deceived me by claiming he had cancer when I tried to end the affair. He violated several codes and may have broken some laws. He's a sheriff's deputy and on the SWAT team. I live in a different county, and he visited weekly in uniform on county time. He does not have cancer, and I now fear him due to his access to many things. Do I have any grounds for a lawsuit?

I am responding to a Summons and a Request for Entry of Default from a Debt Collector for Citibank. My response, whether it be an 'Answer' or a 'General Denial,' requires me to file a Proof of Service form. I am unsure which form to use and believe it might be intended for the Plaintiff, not the Defendant. What form should I use for my situation?
