I am looking for assistance with a possible eviction issue in Wisconsin. My boyfriend (now ex) has been freeloading and living with me for months. I found out he has been cheating and he has my address on his driver's license. I have printed a 5-day notice for eviction and created a voluntary movement of residence form for him to sign, indicating he is voluntarily leaving. I want to know if this form would hold up in court if he signs it and then tries to come back, and if I need to have it notarized or could video tape him signing it.
