My blood sugar has been extremely high, over 500, and I took 25mg of Novolog. How long should I wait before going to the ER? I'm feeling super hot and tired, should I just sleep it off?

I went to the gym yesterday and had a heart rate of 173 while running on the treadmill for 5 minutes, which is higher than my usual 150-160 range. I had sore muscles in my legs, chest, and back, and experienced chest and back pain, possibly due to an uncomfortable bed. I am also always tired and not sleeping well. I was sick two weeks ago and went to the gym four times last week, with a heart rate of 150-160 during warm-up runs. Why was my heart rate so high, and could I have some kind of heart disease?
