I have had mild but sustained chest pain for the last 4 hours straight today. Is this what is known as unstable angina? I am 67 years old and smoke one cigar daily. The pain is not intermittent or stabbing; it was more painful 2 hours ago and is now a sustained, dull ache. Should I go to the emergency room?

I have been experiencing sciatic pain and muscle spasms in my hip, consistent with piriformis syndrome, since 3/7. The pain is constant and includes various types, such as sharp, dull, achy, and stabbing. I had a partial discectomy on L4/L5, L5/S1 in September 2017. I have not had an MRI due to insurance constraints and have tried treatments like lidoderm patches, ice packs, and ibuprofen without significant relief. What should I do next?
