I have a sharp pain in my upper abdominal area under my ribs that makes me feel nauseated. This has been happening for years and comes and goes. Recently, the pain has worsened, and I can feel a bulge. Is this serious, and what could be causing it?

I've been experiencing various symptoms including swelling, pain, decreased volume of ejaculation, foul-smelling urine, and tenderness in testicles. I've been on antibiotics twice and my doctor mentioned a slight rise in white blood count. I also had sores on my chest that looked like chemical burns and took over-the-counter medication for parasites. My regular doctor seems to dismiss these symptoms, and I'm unsure what to do next. What could be causing these symptoms and what tests should I consider?

The report mentions that these structures are parallel to one another and located below the appendix. What type of pelvic fat mass could this be, and what might have caused it?
