My son has a cut on his face. How long has he had the cut? How old is he? Is he currently using any medications? Does he have any relevant medical history? How did he get the cut? Is he up to date on his immunizations?

I've been dealing with nausea, intermittent vomiting, and diarrhea for the past month, accompanied by stomach pain and a headache for the last two days. I'm unable to eat, haven't taken any medication for the headache, and have fibromyalgia, chronic back problems, and a history of stroke in 2004. Should I go to the hospital?

I think I have a problem that needs to be fixed, otherwise it will affect my future. I haven't spoken to a mental health professional about this. I sometimes experience insomnia. Is there anything else in my medical history the Psychologist should know?

I have a hydrocephalus shunt in my head. It has been 7 months since I was diagnosed. Now I am experiencing dizzy spells, headaches, confusion, and memory problems again. I have not seen a doctor yet about these symptoms, which only returned two days ago. Additionally, I have ADHD and my psychiatrist recently switched my medication from 60 mg Adderall to 70 mg Vyvanse four days ago. Could my shunt have failed, or could these symptoms be related to the new medication?

I have experienced two muscular seizures and blackouts over the last three months. How long have you been concerned about the seizure? What are all your symptoms? Have your parents or grandparents had any neurological conditions? Anything else in your medical history you think the Doctor should know?
