What are some reasons a woman might have spotting after being diagnosed with premature menopause several years ago?

I have been experiencing intense itching and burning around my vaginal area, especially during urination, which is so painful that I cry. I have tried using Desitin and Phytoplex Protect Z Guard Paste, but the burning persists. The itchiness comes and goes, and it's worst when I urinate. I have known allergies to penicillin and a medical history of diabetes, high blood pressure, and a stroke. What could be causing this and how can it be treated?
