Do you have to take a combined birth control pill at the same time every day for it to work? Specifically, does taking it one hour late within a 3-hour window affect its effectiveness, especially if it was the last sugar pill of the pack?

I had unprotected sex on 10/27 (3 days after my period ended) and took Plan B within 24 hours. I then had unprotected sex again on 11/9. I experienced light bleeding on/off for 3 days starting on 11/14-11/17, which I'm unsure if it was my period or implantation bleeding. My breasts have been swollen, I've been tired, achy, and peeing more frequently. When should I take another pregnancy test, and could the light bleeding have been my period early?
