I don't exercise but do heavy lifting at my job. I'm not using any contraceptives. What could be causing the light bleeding and should I take a pregnancy test?

I have a vaginal ultrasound scheduled today that I'm terrified for. I had a D&C and Mirena IUD inserted right after about a year ago. Since then, I have been unable to have sex because it is so painful to the vaginal walls. I tried to get the IUD removed but the strings broke, and it was said to be embedded. This experience was so excruciating that I have been terrified to get the ultrasound ever since, due to the pain in the vaginal walls. Is there any way I can ease the pain for my ultrasound today? I need this IUD out as it has caused so many problems physically and mentally.
