I'm not using contraceptives, take venlafaxine for anxiety/depression, iron supplements, and vitamin B12. What could be causing this and what should I do?

I have been bleeding between my periods for a couple of months with severe pain during my cycle. The bleeding between periods is light and constant, with no pain. During my cycle, I pass clots. The pain is localized in my abdomen and back, feeling like someone is twisting my abdomen, and I experience nausea and bloating when the cramps worsen. What could be causing this and what should I do?
