I have suddenly started having severe pain in my abdomen and pelvic area. The pain started yesterday after I woke up and lasted over 3 hours, with the first hour and a half being the most severe. The pain comes and goes, especially when I use the bathroom or cough, and feels sharp on one side of my pelvis, with cramps at the top of my abdomen and pressure pain in my pelvis and anal region. I took Tylenol yesterday, which was my first time experiencing this. I am 41 years old, had my last period on May 1st, and am not on any birth control. I have had one pregnancy with a living child, but I don't know if I have had miscarriages. My last gynecological exam was a few years ago. There is a family history of fibroids from my mother's side.

I have an IUD for birth control, and today after a mile walk, I experienced an irregular amount of blood discharge, followed by more blood, blood clots, and weird tissues later. I've had the IUD for 4 years, my last period began on March 10th, and my last pap test was a year ago with negative results. I'm concerned about a possible miscarriage. I'm not on my period, but I keep seeing blood come from my vagina, especially during sex, even though my partner is very gentle. I recently had a UTI which I've cleared, and I'm on birth control (Tri-Lo-Mili, 0.25mg). My last period started on September 22 and lasted for about 5 days, but the bleeding has continued in a brownish form. I feel perfectly fine with no redness, bumps, sores, itchiness, discharge, or cramps. I am sexually active with one partner who tested negative for STDs. My last pregnancy test was a few weeks ago and it was negative. I haven't had a pap smear or STD screening yet. What could be causing this unusual bleeding and what should I do?

I am experiencing intermenstrual bleeding that has been going on for a week. I am 20 years old and usually have regular periods.
