I'm 44 years old and have been experiencing on-and-off heavy bleeding with dime-sized clots since April. I have a history of dysplasia and have had five children, the last one in 2013. I am not on any medications except for sinus medicine and have not undergone any tests or examinations. I experience fatigue and sometimes lower abdominal pain or cramps. What could be causing this bleeding, and what should I do?

I had a full abdominal hysterectomy back in 2009 and have been on Premarin since. Premarin helps with my hot flashes, but I've been crying very easily since the surgery. I am experiencing lower abdominal pain immediately following intercourse. The pain is persistent, sharp at first, and turns to a stabbing sensation when I move. I also feel nauseous and bloated.
