I had insertive anal intercourse with a condom breakage. I started Truvada within 24 hours as PEP. I later learned that a third drug is typically added to this regimen, but I was not prescribed it. Is a third drug critical for PEP to work, and if so, is it too late to start it now? Or would starting it now still make sense having started Truvada on time? Additionally, I'm experiencing unpleasant GI effects from Truvada, such as persistent diarrhea. Should I consider adding a third drug despite the side effects and the late start?

I have been spotting for a few weeks, and today I experienced a heavier blood flow resembling a menstrual cycle, which I should not be having as I am on birth control. Additionally, I developed a small lump at the 5 o'clock position outside my rectum this morning, which is slightly uncomfortable but not painful. How long have you used birth control? Has the spotting been stable or getting worse? Are you using any medications, either for the spotting or anything else? How old are you?
