What would make a doctor that performed an open myotocomy to remove a fibroid not to remove a polyp as well? Is it related to insurance, sedation time, or the doctor's professionalism?

I have been bleeding and passing large blood clots for 7 days, with the bleeding getting heavier each day. My period never fully came about 2 weeks ago, but I was lightly spotting dark brown blood and passing some dark brown tissue. I am not using any contraceptives and had a tubuligation done in 2011. My last pap test was about 2 years ago with no unusual smells except up to 2 days after sex. I have been doing a cleanse and my thyroid level was recently low but is now back to normal. Could this be related to my current condition, and what should I do?
