I've been on Mirena since September 2010. Why am I bleeding every day and experiencing cramps? My gynecologist checked it last week and said it should be in place, but should I seek a second opinion?

I had a uterine biopsy on Tuesday. By Wednesday, I started having headaches, which worsened by Friday, leading me to go to the ER where a CAT scan found nothing. It is now Sunday, and I have an enlarged lymph node in the right groin area, along with ongoing headaches, shaking, chills, a temperature of 99.3, cramping, a pulling sensation in my right side, and tenderness. I also have a tender spot at the upper right labia majora, possibly an ingrown hair or a pimple, and I have HPV. My last menstruation period was a year ago, and the biopsy was done due to post-menopausal spotting. An ultrasound showed a large number of fibroids and a very small cyst on the left ovary. Is this swollen lymph node something that warrants a return to the ER, or is it something that can wait until I can get an appointment with my GYN?
