Why would spotting in the middle of a cycle only start in my mid-20's, happening 4 cycles in a row now? Is there something wrong possibly?

I am taking Necon .5/35 birth control pills and consistently take them at 1 pm. Today, I took my pill 45 minutes late and had sex relying on the birth control as my contraceptive method. I also want to understand the difference between combination hormone pills and progestin-only pills, and why the directions say to take the pill at the same time each day if it's not strictly necessary.

I’ve had a right pelvic ache that radiates to my lower back at times, it feels like my ovary but then it doesn’t, I have a bad coccyx sacral spine but I don’t know if it’s originating from that. I have no pain when I sleep, it starts up when I start walking and moving, my hip doesn’t directly hurt, I can move my hip and leg freely, my periods are normal, no abnormal discharge or bloating, It’s a sore ache from lower back to right lower pelvic area groin area. Some days I’m fine the next it’s bothering me. What do you recommend?

I'm 22 years old, my breasts have been sore, I've been tired, and I took Plan B on October 28th. I took a pregnancy test, which was positive. How can I determine how many weeks I am, and is there a reason I got my period twice in November?
