Even though a corporation has profits or losses, do their shareholders receive a 1099-DIV form? If a company has a profit of $8000.00, will the 1099-DIV form show dividends for that amount since I'm the only owner?

I have several 1099-DIVs that contain both ordinary and qualified dividends. Are the qualified dividends separate and distinct from the ordinary dividends, or are they a subgroup of the ordinary dividends? I ask because I have one 1099-DIV that shows both ordinary and qualified dividends with equal amounts (i.e., $1,041.13). This seems too much of a coincidence.
