I funded my Traditional IRA with after-tax money in January 2022 and converted it to a Roth IRA in March 2022. I received a 1099-R for 2022 with amounts in box 1 (gross distribution) and box 2a (taxable amount). I believe this conversion should be nontaxable. Why is there a taxable amount, and how should I report it on my 1040?

My husband rolled over roughly $1500 from one 401k to another 401k. During the process, the check to the new 401k was lost in the mail. The 401k (Fidelity) issuing the check is putting a stop on the check and issuing a new one to T. Rowe Price (New 401k). However, our 1099R is making it look like we took the money out as 'income'. What options do we have to complete our tax return and not be penalized for this error?
