I need the fax and phone number to resubmit a beneficiary recontact report form 1588. I have already mailed it twice and was given these numbers to fax and call after faxing to confirm receipt. However, I spilled liquid on the paper with these numbers and can't read all the digits. Can you provide the correct fax and phone numbers for this purpose?
