I am opening a TFSA account with Questrade and need to fill out a W-8BEN form for foreign withholding tax purposes. I am unsure about what to write in Part II for claiming tax treaty benefits and if I should leave certain lines blank. Specifically, I need guidance on lines 5, 6, 7, 9, and 10 of the form.

I worked under a 1099 for a company in 2019 and requested my tax information at the end of the year, but it was never provided to me. I requested it again last year and still did not receive it. Now, I'm being held up on my tax return for this year because the earned income hasn't been reported. What should I do: request the tax information again or report to the IRS that the company is withholding my tax papers?

On my W-2 statement, Box 2 lists $4886.50 and Box 17 lists $1473.83. In the beginning year of 2020, I claimed '0' so that IRS takes it all out. Will I see all of it back now, the $6360.33?
