My dog, Cutie Pie, had a gasping breathing attack this morning and does not want to walk too far. The temperature outside is in the high 70s. She has a granuloma on top of her paw and licks the underside of both front paws, a habit she has had for years. She is 9 years old, 65 lbs, and had major surgery when she was 3 months old due to a hernia where her internal organs went up to her chest, but has had no complications from that. What could be causing her breathing issues and reluctance to walk, and what should I do?

My 5-year-old toy poodle has had constant ear itching for 3 years despite using Ottomax, visiting the vet multiple times, and trying various natural remedies and medications. The ears are red with tiny black dry spots, but there are no ear mites or fleas. The dog is on heart guard and has never had a flea.
