My 15-year-old dog has suddenly developed hot spots, despite taking tacrolimus ointment daily for SPK in his eyes, eating high-quality dog food, having normal labs including a thyroid panel, and being free of fleas or parasites. The vet diagnosed him with OCD and prescribed anti-itch medication, but there has been no improvement after a week. What could be causing the hot spots?
