This morning, after a tea tree bath, he seemed okay but later became restless, hot, itchy, and vomited. I'm worried about him. Should he go to the vet?

I volunteer at a dog rescue in Mexico. One of the puppies had a severe reaction after his second distemper vaccine, including salivation, pale mucus membranes, lethargy, and a temperature drop. We administered Diphenhydramine and he recovered. He is due for his third vaccine. What are the chances of a similar or worse reaction, and should we premedicate him before administering the vaccine?
