I have a mix XXXXX XXXXX/terrier and she has black scabs under her white coat. When I rub her, hair comes out with black stuff on the end. Her hair on her back around her neck is thinning. What could be causing this and what should I do?

My Yorkie, who is 6 lbs, has hives on her tummy. We were in the ER 2 days ago, and she was clear yesterday, but they appeared again last night. She is not sleeping because she is itching and scratching, and I am not sleeping because I want to make sure she is breathing. In the ER, they gave her a shot of Benadryl and a steroid, which worked immediately. Why do the hives come back at night?
