My 18-year-old dog has been experiencing urinary incontinence for a long time. She recently went piddles outside and was praised, but now she's just dribbling in her diaper and not acknowledging she needs to go outside. We've tried medications and cranberry pills, but she hasn't gone piddles in two days. What could be causing this and what should I do?

I'm visiting a shelter in Serbia with 400 dogs where there's an outbreak of distemper affecting about 20 dogs. Should the other dogs be vaccinated immediately or after the outbreak has been dealt with? There's no means of separating the infected dogs.
