I am a psychologist in Pennsylvania with a non-compete clause in my independent contractor agreement that restricts me from working within a 5-mile radius of my employer's practice. I want to know if there is a standard form to submit to the Court to file a declarative judgment request to nullify or amend this clause, considering the owner is threatening to sue if I move to a location within the restricted area.
