Is it healthcare fraud if a clinician is required to add their diagnosis to a list of previous diagnoses of a patient, sign their name and credentials, and not change the previous diagnoses, even if the clinician does not agree with the diagnoses listed and does not know where the previous diagnoses came from?

My 16-year-old daughter is being discriminated against at her Catholic High School due to medical issues, specifically migraines. She has been told she needs to leave the school for academic reasons, despite being respectful and following school rules. She misses a lot of school due to her condition and is always trying to catch up. The principal and a specific teacher are harassing her, and I have evidence of false statements and unfair grading. Is there any legal recourse or case for this situation?

I bought a vitamin store in Kissimmee, Florida in January 2010 through my corporation, 'Far Horizon.' The sellers and broker presented the store as wildly profitable, but it was actually losing money due to their fraudulent practices. I gathered evidence and sued them, but my attorney resigned a month before the trial. The defense attorney got the judge to dismiss my corporation as a plaintiff with prejudice, and now seeks partial judgment on three counts. The argument is that I, as an individual, was not a party to the Closing Agreement, Covenant Not To Compete, and Bill of Sale Agreement. I have two days to amend my complaint. How can I rewrite the three counts to reflect that I was the sole funder and decision-maker for the corporation?
