Can I use hydrogen peroxide 3% to clean a deep wound?

Does this bump on my breast look like something I should go to the ER for? I am 29 years old and have been experiencing pain, tenderness, itching, swelling, and pus discharge from the bump for approximately 3 weeks. I also have generalized body aches, diarrhea, and am currently taking Cephalexin. I have hypothyroidism but my levels are within normal range, and I recently tested negative for Covid.

I have two issues: 1. When I breathe in deeply, I smell ammonia at the back of my nose and it feels like it burns a little. I don't have kidney disease and my recent CT and kidney function tests were normal. 2. I have bleeding from just outside my anus that burns. What could these symptoms indicate?

Now, I need a doctor's note from January 21 to February 16 to return to work. What can I do to manage this condition and obtain the necessary documentation?
