I have severe pain in my lower back that started yesterday and is constant. The pain is sharp and doesn't radiate down to my legs. I have tried taking 400 mg of ibuprofen every 4-5 hours, but it doesn't seem to help. What could be causing this pain, and what treatments can I try?

I have been experiencing diarrhea for the last 3 weeks on and off. I am diabetic and take 1000 mg of Metformin daily. My blood sugar levels have been running between 117 to 130 for the last 3 weeks. I have a persistent issue on my penis that multiple doctors have diagnosed as dermatitis, but I'm still worried. I've been using hydrocortisone cream, but it seems to dry the area out more. There's a small white spot that I'm particularly concerned about, and it appears to be getting slightly bigger. The area is now smoother but still rough to the touch. What could this white spot be, and is there anything else I should do or consider?
