I am an 82-year-old diabetic with well-controlled blood sugar for 15 years using metformin and glimepiride. However, my blood sugar levels have recently started to climb, and despite increasing the dosage and maintaining a strict diet, my morning readings are consistently high, ranging from 190s to 210. My doctor has run tests and found no issues, but I am reluctant to start insulin. Is there any other medication that could help lower my blood sugar before resorting to insulin?

My father had an epidural hematoma which started with a massive nosebleed and after surgery left him paralyzed. He had no nosebleeds for 18 months, but then they started again, with three in 10 days. His blood pressure is fairly normal at 130/80, and his INR level is believed to be normal.
