Can someone help me determine if I have a UTI, kidney stones, or both, given my symptoms of moderate blood, positive nitrates, lower back, flank, and groin pain, drops of blood at the end of urination, burning sensation, and a history of passing a kidney stone?

I have been experiencing severe itching, biting, and crawling sensations on my skin, along with black dots that appear when using a lint brush. I have been treating the symptoms with permethrin and ivermectin, which initially worked but require reapplication. I also have swollen legs and ankles, which I never had before. I am currently on suboxone to mitigate opioid withdrawal symptoms, but I am concerned that it might be causing or exacerbating my skin issues. What could be causing these symptoms, and how can I address them?
