I am having difficulty eradicating small bowel bacterial overgrowth even after antibiotic regimens. I have had several H2 breath tests which continue to indicate very high levels of methane but virtually no hydrogen. I am a well-controlled type 1 diabetic for 24 years with an average A1c of 6.0 and no complications. I have mild fat in stool, slightly high but not significantly out of range, and bowel movements are consistently on the softer side. What has to be done to successfully get this accomplished, and what are your impressions and thoughts?

I have severe insomnia and have tried Ambien, which works half the time. I recently started taking Trazodone, but I'm unsure about the best dosage and timing. Some suggest taking it 2 hours before bed, while others say 30 to 90 minutes before bed or even right at bedtime. I've been taking 50mg for two weeks, but I only sleep about 2 days a week for 3 hours, and on other days, I don't sleep at all. What is the correct dosage and timing for Trazodone to help with my insomnia?

I have been suffering from severe back pain and fibromyalgia for over a decade, and narcotic pain medications like Lortab 10/500 are the only effective treatments besides exercise. My primary care physician is uncomfortable prescribing these medications regularly and has referred me to a physiatrist. I live in the Boston area and am concerned about the difficulty in obtaining these medications due to DEA regulations. Will the physiatrist prescribe these medications, and if not, are there sympathetic doctors or clinics in the area that might?
