I am reviewing an Echo report from 2016. I have reduced pulmonary functioning and my Pulmonary doctor asked if I have had an echo. I noticed my left atrium was slightly dilated. I am unclear about the reference tables for left atrial volume index (LAVI), which suggest different cutoff values for concern. What drives the conclusion of above or below 34 ml/m2 being something to watch or be concerned about? Also, my inferior vena cava (IVC) was reported as normal but decreased less than 50% with respiration, while I read it should be greater than 50%. Should I request a follow-up? Heart disease runs in my family, and I am a carrier of Alpha 1 Antitrypsin defect.

I break out in hives at night, usually between 2-4am, mainly on extremities/hands, sometimes on the torso. This wakes me up and causes intense itching. I take Benedryl to stop the itching. I am 58 years old and this has been happening for 2-3 months. There have been no changes in detergents or cosmetics. I have been experiencing continuous head spinning for two months, diagnosed with left ear vestibular dysfunction or Meniere's disease. I have tried various medications including water pills, meclizine, and betaserc 16 mg twice a day, but they have not improved my condition. I am also taking blood pressure medicine and anxiety medication. My MRI, blood tests, and heart check-up are all clear. I feel unbalance and my head and eyes spin constantly. What could be causing this, and what is the best treatment to relieve my symptoms?
