I am a 34-year-old female experiencing severe fatigue, bloating, and unexplained weight gain (approximately 12lbs in 2 months). I have normal periods, some headaches, and no known medical illnesses. I tried two antidepressants, which led to weight gain, and my stomach now looks like I'm about 3 months pregnant. My thyroid and blood tests came back normal. I have CTS in both hands and two nights ago I fell asleep with my right hand tucked at the wrist. I woke with numbness starting at the knuckles (top side) and going all way up my arm past my elbow. There is no numbness on the underside of my arm. My thumb is numb but my fingers are only slightly numb. My wrist is completely turned downward as are my fingers. I am completely unable to raise my wrist or fingers upwards and it is difficult to move them. I can't use my hand at all, though I touch my thumb to my index finger but cannot press. My hand is just hanging at the wrist and no matter how hard I try to raise it, it's totally impossible. When I bend my arm at the elbow downwards it is painful but no other pain is present except below my shoulder, when touched is very painful. I also have fibromyalgia and myofascial pain syndrome in my other arm. My symptoms are worse today than yesterday. What did I do to myself while sleeping? Also, my thumb is getting more numb.

My 79-year-old dad has a long-standing complaint of difficulty breathing. Despite numerous visits from paramedics, consultations with doctors, an operation to dilate the esophagus, asthma inhalers, and throat lozenges, the issue persists. The medical team now suspects it might be psychological due to his dementia, but he is genuinely distressed when the throat constriction becomes severe. He has difficulty swallowing and is frequently woken at night by this issue. What could be causing this and what further tests should be considered?
