I have been experiencing congestion in one nostril that alternates sides, along with head pressure and lightheaded feelings on and off throughout the day. Could this be due to a change in weather, a tension headache, or something more serious?

I am 72 years old with diabetes type 2, taking metformin and trulicity. I have long COVID-related inflammation on my left side and sporadic pain on my left foot near the toes, which feels like it could be gout or an infection. The pain is tender and feels as though something was dropped on it, but there was no impact. I have been experiencing this since July 22. I also have a history of using metformin since 2004, hydrochlorothiazide, losartan, levothyroxine, a multivitamin, and significant pain on my left side, including cervical spine issues and torticollis. I am not currently seeing a doctor for these issues.
