I have been experiencing small amounts of bright red blood on toilet paper when I wipe, along with extreme itchiness and occasional pain after wiping. My bowel movements vary from large and big to very ribbon-like. I am a 26-year-old moderate smoker and drinker, with no other drugs or medications. This bleeding has been occurring for about two months, with no pain during bowel movements, but pain when wiping and after. I also have a 103.5 fever today, which I doubt is related. What could be causing these symptoms, and what steps can I take to address them?

I’ve had RUQ (right upper quadrant) pain for almost 2 years, with constant but fluctuating intensity, located under my right rib cage. Additionally, I’ve experienced back pain in the same area for the past 2 months, which worsens when pressed. I’ve undergone an ultrasound, HIDA scan, and CT scan, all of which showed no abnormalities. My blood work has also been normal. What could be causing this pain, and what should I do next?
