I have severe kidney pain for about 14 hours, which is located under my lower ribs and lower side back. I have taken only Panadol and visited the hospital last night, where scans showed no issues. The pain has shifted towards the front and feels like a stitch. What could be causing this pain?

I have been taking 100 mg tramadol every 6 hours for 7 years, but it has become ineffective. My pain management doctor has switched me to Norco 5/325, 3 times a day. I work 8+ hours a day and need 4 doses a day to manage my pain, but my doctor only prescribes 3. My pain is localized in my hand and wrist due to an old trauma injury, and it is mostly a constant ache with sharp, stabbing pain at times. I am also on Gabapentin, but I'm unsure if it's effective. Why am I being treated like this, and what can be done to manage my pain more effectively?
