My 58-year-old brother has had swollen feet, legs (below the knees), and hands for the past 5 days. He is also having trouble catching his breath. He is on a mild antidepressant for the last 3 months. What could be causing this and what should be done?

I just received blood test results showing a Ferritin level of 639. How serious is this, and how soon should I donate blood? I'm a 63-year-old male, and the rest of my blood test results were fine, including normal liver enzyme levels. I had a high Ferritin level two years ago and was advised to donate blood, which I did, and the level went down. I'm not sure what the level was at that time, but I'm wondering how dangerous the current level of 639 is.
